Appointment Information

We see patients on an appointment basis. New patient visits and annual eye examinations may require an hour;

follow up visits may require 15 - 30 minutes.
At your first visit you will be asked to complete a medical history and registration form.

Please prepare to give your insurance information when calling.

 

Bring to your first visit:

  • An up-to-date medical and/or vision insurance card

  • Your current eye glasses and your written lens prescription from your
      previous doctor, if possible

  • A contact lens case if you wear contact lenses

  • Any current eye examinations

  • A list of any other current medications and dosages you are currently taking

  • A pair of dark glasses to comfort your eyes after dilation

 

Parking Registration:

When you call to make an appointment, please be ready to give us your vehicle license plate number, so we may register your vehicle for parking.

 

As of January 1st, 2019, the property management has begun monitoring parking due to increasing parking violators from surrounding businesses.

Eye Examinations
We provide Medical eye examinations and Routine (yearly) eye examinations.
Contact Lenses

Contact lens fittings are separate and outside the realm of a standard, complete eye examination and refraction. We will prescribe and fit you with contact lenses if you so desire.
