Fees & Insurance

Fees and Payments:

Charges for office visits and operations are based on the complexity of the service as well as the time required for your care. If you lack vision or medical insurance we ask that you pay for your visit in full at the time of service.

We accept cash, check, and credit cards (Visa, MasterCard, & American Express) for payments.

Insurance & Referrals:

If you have vision and/or medical insurance, we ask that you pay any copay and/or deductible at the time of service.

Eyeglass prescriptions (refraction) are covered under your vision coverage, and vision coverage is often separate from medical insurances. Please contact our office to verify your vision coverage.

 

Dr. Morton is a participating provider for most major insurance programs. We also accept Medicare assignment. Our office will submit the claim to your primary and secondary insurance carriers. You will receive a statement for any remaining balance due after your insurance has responded.

If your insurance carrier requires that you obtain a written referral from your Primary Care Physician (PCP) to be seen in our office, please request this well in advance of your scheduled appointment. Sometimes it takes 1 - 2 weeks to receive. If you have an emergency and need to be seen the same day, you may need to go to the PCP's office to obtain a written referral to bring with you to the appointment. Or the PCP may fax the referral directly to us at 425-562-9085. However, we MUST have the written referral before you can be seen.
